ECFMG Certification for IMGs: A Step-by-Step Guide (2027 Match)

If you graduated from a medical school outside the United States and Canada, there is one credential you must have before you can start a US residency: ECFMG Certification. Without it, you cannot enter the Match as an independent applicant, start graduate medical education, or take USMLE Step 3.

When I came to the US in 2003, the process looked very different. Step 2 CS no longer exists, and the rules change almost every year. This guide explains the current requirements in plain language, step by step.

Last updated: October 2026. ECFMG updates its rules every year, so always confirm details on the official ECFMG website before you apply.

What is ECFMG Certification?

ECFMG (Educational Commission for Foreign Medical Graduates, now part of Intealth) checks that international medical graduates (IMGs) are ready to enter US residency. It confirms three things:

  1. Your medical education is real and meets its standards.
  2. You have the medical knowledge (USMLE Step 1 and Step 2 CK).
  3. You have the clinical and communication skills (an ECFMG Pathway plus the OET Medicine English test).

Step 1: Check that your medical school qualifies

Before you spend any money, go to the World Directory of Medical Schools (wdoms.org) and look up your school. Open the Sponsor Notes tab. Your school must have an ECFMG Sponsor Note, and your graduation year must be covered by it. If it is not, you are not eligible for ECFMG Certification.

You also need credit for at least four years of medical school from a listed school. Transferred credits have special rules.

Step 2: Open your MyIntealth account and apply for certification

Create an account in MyIntealth (ECFMG’s online portal) and submit the Application for ECFMG Certification. This tells ECFMG who you are and where you studied. You will need a notarized identity form and a photo.

Step 3: Get your medical diploma and transcript verified

ECFMG contacts your medical school directly to verify your final medical diploma and requests your final transcript. This is called primary-source verification. Some tips:

  • Contact your school’s registrar early and tell them to expect the request. Slow schools are the number one cause of delays.
  • If a document is not in English, include an acceptable English translation. Otherwise ECFMG translates it for a fee (currently $52 per page).
  • Never submit altered documents. ECFMG can make a permanent “irregular behavior” note on your record.

Step 4: Pass USMLE Step 1 and Step 2 CK

These are the medical science exams. Step 1 is now pass/fail, so your Step 2 CK score has become one of the most important numbers on your residency application. Aim high.

You register for these exams through ECFMG and can take them at Prometric centers in the US and many other countries. Check the USMLE website for current fees and any international testing surcharge.

Watch the 7-year limit: all exam requirements must be met within seven years of the first exam you pass. Plan your timeline so your Step 1 pass doesn’t expire before you finish.

Step 5: Pass the OET Medicine English test

Every applicant must pass OET Medicine, even native English speakers. ECFMG says there are no exceptions. For the 2027 Pathways you need these minimum scores, all in one sitting:

  • Listening: 350
  • Reading: 350
  • Speaking: 350
  • Writing: 300

If you miss the minimum on any part, you must retake all four parts. For the 2027 Match, the test must be taken on or after January 1, 2025, and no later than the last OET test date in December 2026.

Step 6: Complete a Clinical Skills Pathway

Since Step 2 CS ended in 2021, IMGs show their clinical skills through one of six Pathways:

  • Pathway 1: You hold (or recently held) a license to practice medicine without supervision.
  • Pathway 2: You passed a clinical skills exam (OSCE) required for licensure at an acceptable medical school.
  • Pathways 3, 4 and 5: For students and recent graduates of schools that meet specific accreditation or degree criteria.
  • Pathway 6: For everyone else. Licensed physicians observe you with six real, in-person patient encounters and grade you using ECFMG’s Mini-CEX. Telemedicine and standardized patients are not accepted.

Most practicing doctors from abroad qualify for Pathway 1. If you previously failed Step 2 CS, you must use Pathway 6.

The Pathways application fee is currently $945.

Key deadlines for the 2027 Match

  • January 31, 2027: Submit your Pathways application.
  • January 31, 2027: ECFMG must receive your OET scores directly from OET, and any required documents.
  • NRMP Rank Order List deadline: You must have passing Step 1 and Step 2 CK results by this date to stay in the Match.

Miss the January deadline and you will not be eligible for the 2027 Match, so don’t leave it to the last week.

How long does it take?

Most IMGs need 1 to 3 years from starting Step 1 preparation to full certification. The slowest parts are usually studying for the exams and waiting for your school to verify your documents. Once all requirements are met, ECFMG usually issues the certificate in about two weeks.

My advice from experience

  • Start the paperwork first. Verify your school and documents while you study, not after.
  • Treat Step 2 CK as your main score. With Step 1 pass/fail, programs look closely at Step 2 CK.
  • Book OET early. Test dates fill up, and the January deadline is strict.
  • Certification is only the beginning. To match, you also need US clinical experience, strong letters and a realistic specialty choice. I passed my exams with high scores but applied only to dermatology and didn’t match. Don’t make the same mistake.
